Please see below the permission to administer medicine and pre-existing injuries form.
Please print and complete the forms below if your child requires medicine during their time with us or has a pre-existing injury. You must bring this form on your first day and hand in to the Community Activities office along with any medication. Please write your child's name on the medication.
Inhalers and Epipens must be named and placed in the medical box in the Community Activities office upon arrival. If you have not brought the appropriate medication required for your child he/she will not be able to participate until the medication has been delivered to the office.
Please inform us at the time of booking if your child has any dietary requirements or food allergies. We request that children and parents do not bring any food containing nuts on site. For more information on food allergies, and the 14 key allergens please see the Food Standards Agency's website.
Please speak to a member of our team if you have any questions concerning what to pack in your child's lunch box.
